For this I will pay the listed rates currently offered during the time of this contract for they number of days/nights for boarding and or any additional Board & Train Programs I choose or select below.

You agree to pay the total remainder fee due for this stay; payable in full by cash, or check made to Team Floppy Ears LLC, or Zelle to: teamfloppyears@yahoo.com by or on the drop off day or by pre-approved payment plan. For the fee identified on page 1 of this agreement, the dog(s) will be boarded with Team Floppy Ears, LLC. for boarding or board and train services as discussed for the stated time period, have access to fresh water at all times, and will be fed their normal dog food for the duration of their stay. The boarding dogs will be thoroughly exercised for a minimum of two hours every day and, if deemed dog friendly, they will be able to socialize and play with other dogs at Team Floppy Ears, LLC.’s house throughout the day. The boarding dogs will have shelter and regular human contact and will be treated well while in the care of Team Floppy Ears, LLC.

Team Floppy Ears, LLC. wants to make sure your dogs are well and we will do all we can to keep them safe and healthy while they are in our care. We do, however, know that unexpected events can occur, and as such, want make sure we fully understand how you would like us to respond to such an unlikely emergency event. By default, we will take any and all actions to save animals lives and provide medical treatment at the closest veterinary hospital. The following statement outlines how this could financially impact you and gives you the opportunity to direct us differently should you so desire. I understand that I will be solely financially responsible for any medical expenses my dogs may incur in case of emergency treatment of any kind that might become necessary while my dogs are in the care of Team Floppy Ears, LLC.. I explicitly authorize Team Floppy Ears, LLC. to take all necessary measures to treat any and all injuries at the closest veterinary hospital should such action become necessary for any reason. I explicitly authorize Team Floppy Ears, LLC. to direct any veterinary services to save my dog’s life and make medical decisions in my Dog Boarding Contract with Team Floppy Ears, LLC. www.teamfloppyears.com Page 6 absence if necessary regardless of cost and understand that I will financially liable for any and all such expenses.

Team Floppy Ears, LLC. and associates are very experienced at handling dogs of all temperaments and have a solid understanding of dog behavior. We will never expose your dogs to any situation that we feel could be a danger to them or others and we will take all reasonable measures to assure their safety while they are in our care. However, dogs are animals and as such react by instinct and not rationality. Dogs that generally behave well can still get involved in altercations when the wrong sets of circumstances come together. We are sure you understand that we can not assume responsibility for your dogs’ actions, especially should they lead to harm to other animals, people or themselves. Owner understands that Owner is boarding dog(s) in an environment where they will be socializing and interacting with people and other dogs of all sizes, after proper evaluation of course. As always, with the interaction of dogs, if any, there is a chance of injury. Owner assumes all risks of injury to Owner’s dog(s) while at Team Floppy Ears, LLC.’s house or in transportation to any veterinarian clinic, so long as reasonable care is taken to prevent any unnecessary injury, death or loss. By boarding dog(s) with Team Floppy Ears, LLC., Owner agrees not to file legal claims against Team Floppy Ears, LLC. or any employees for any injury, death or loss of Owner’s dog(s) (unless such claims are based on the reckless or intentionally harmful actions of Team Floppy Ears, LLC. or associates). Owner hereby waives and releases Team Floppy Ears, LLC. from any liability of any nature for any injury, death or loss of Owner’s dog(s) resulting from Team Floppy Ears, LLC.’s actions (unless such actions were reckless or intentionally harmful) or from the actions of Owner’s dog or any other dog while in the custody of Team Floppy Ears, LLC.. In the event my dog causes injury to another dog or to a person while with Team Floppy Ears, LLC., Owner agrees to indemnify Team Floppy Ears, LLC. in any action which may be brought against it and for any defense, settlement, or judgment against it. Owner will assume all liability for the actions of Owner’s dog(s). Team Floppy Ears, LLC. and the Owner agree to resolve any dispute that may arise between them (including, but not limited to, disputes concerning the interpretation, alleged breach or enforcement of this Agreement; Team Floppy Ears, LLC.’s care for the Owner’s dog(s); any alleged reckless conduct, intentional misconduct, or other tortious or wrongful conduct on the part of either party) through binding arbitration before a single, neutral arbitrator pursuant to the rules of the American Arbitration Association. The arbitrator shall apply the laws of the State of California, without regard to conflict of laws, in any dispute between the parties. If a dispute arises between the parties, each shall have the right to seek all necessary and proper relief, excluding exemplary or consequential damages. Consistent with applicable law and his or her discretion, the arbitrator may award reasonable attorney fees and costs (including fees charged by the arbitrator) to the party prevailing in any arbitration proceeding. Owner understands and agrees that he or she is waiving his or her right, if any, to a trial by jury in any dispute with Team Floppy Ears, LLC..

I certify that I am the owner or the agent of the owner of the registered pet(s) and that I am authorized to board the pet(s) and sign this form. I authorize Team Floppy Ears, LLC. to contact my veterinarian in order to confirm health, temperament, and vaccinations. I give consent to Team Floppy Ears, LLC. to act on my behalf by obtaining veterinary care at my expense, should Team Floppy Ears, LLC. deem it necessary. Due to the many outstanding benefits of dog socialization and Team Floppy Ears, LLC.’s commitment to the safety and well being of my pet(s), I agree that the benefits of socialization outweigh the risks described above. Furthermore, I request a socialized environment for my pet(s) while under the care of Team Floppy Ears, LLC.. I have read the schedule of fees and agree to pay all charges at check-out/drop-off, unless previously agreed otherwise in writing. I authorize Team Floppy Ears, LLC. to charge my credit card account, if so provided, for any outstanding invoices that remain unpaid 14 days after boarding has ceased. In addition to releasing Team Floppy Ears, LLC. (and associates) from any liability or legal claim due to injury or death of my dog, unless Team Floppy Ears, LLC. has been reckless or intentionally harmful in the care of my dog, I understand that under no circumstances will Team Floppy Ears, LLC. be liable for exemplary or consequential damages or damages beyond the replacement value of my dog. Should Team Floppy Ears, LLC. incur veterinary costs for emergency treatment as described more fully above, in Team Floppy Ears, LLC.’s sole discretion, such amounts shall be paid by Owner to either Team Floppy Ears, LLC. or the persons or entities providing veterinary services.
